School Counseling Forms
Juniors and seniors are given three days per school year to tour college campuses. These college days must be approved by each of their instructors. The request form must be submitted five days before the requested absence.
Transcript release form must be completed by the student and submitted to the counseling office.
Students who wish to add or drop an elective may do so during the first week of the new school year. After the first week of school, no schedule changes will be made without teacher recommendation and administrative approval. Required courses cannot be dropped. Permission to add/drop any class is contingent upon course enrollment.
Students must complete 24 credit hours to graduate from Fayette Academy. State course requirements must be fulfilled in specified areas.
Fayette Academy encourages students to plan their secondary coursework to reflect postsecondary goals. Included is a proposed framework for a high school academic plan.
